Flu Clinic for District Students

posted Jan 3, 2018, 2:39 PM by Russell Florez   [ updated Jan 3, 2018, 2:39 PM ]

On January 24th, the district nursing staff is providing another and final flu clinic for district students. The nurses will be administering the injectable flu vaccine only.  The flu shots will be available on a first come, first served basis.  The clinic will be held at Bloomfield High School from 8:30 to 12:00.  There is no school this day due to the Professional Development Day.  You will need to make arrangements for your child to get to the high school.

The consent form and information about the flu shot clinic is available at Department:   Health Services.   If you cannot print the consent form, copies can be obtained from the school nurse or the main office of your child’s school.  Please send completed consent forms to the school nurse by 1-23-18.  You may walk in to the high school with your child on the 24th to see if any vaccine remains.  Walk-ins will be immunized if vaccine is still available.  You may call the high school nurse office on 1-24-18 between 8:00 and 11:00 to check on vaccine availability (634-3498).

Please complete the consent form carefully and completely, and return the consent form to the nurse by January 23rd.   Incomplete forms will be returned with your child; incomplete forms may result in your child not receiving the flu shot.

You may call your child’s school nurse if you have any questions.
